CVI/Functional Vision Evaluation Kit

CVI FUNCTIONAL VISION EVALUATION MATERIALS

(To use in addition to materials in a standard FVE kit)

  • High powered flashlight, 250 lumens or greater (such as UltraFire Mini Cree)
  • Solid colored squishy toys to fit on end of flashlight (or colored caps from APH)
  • Mylar Pom Poms (red and/or yellow and lighted) and other reflective toys and targets (mylar balloons, tin pie plate, mirror, etc.)
  • Slinky Jr. by Poof (in red and/or yellow or in student’s preferred color)
  • Lighted Slinky or illumi-spring (see strategytosee website DIY section)
  • Red puff balls in various sizes (for observing near and distance functional vision)
  • Small, edible foods to use as targets (Skittles, Cake Decors, Cheerios) or student’s favorite foods
  • Illuminated or light up cup, cutlery, bowl and toothbrush
  • Mardi Gras Bead Curtain (see strategytosee website, DIY materials)
  • “Yellow Spot, Yellow Spot, Where Are You?” an informal game for observing near functional vision
  • CAD Charts (for observing difficulties with Complexity At Distance)
  • Color photos of real targets and black and white line drawings
  • Colorful Gel Window Clings and/or Swirly Mats for use with Light Pad
  • LightAide
  • APH Light Box or commercially available light pad such as an AGPtec LED Light Pad
  • Glow Crazy Doodle Dome or black pop up tent to use as a CVI Den (see strategytosee website, DIY materials)
  • Variety of lights and lighted objects including battery operated puck lights in soft white and variety colors
  • Metal reflective mixing bowl with light up targets inside, such as red Thumb Chucks
  • Variety of yellow and red toys such as Elmo, Big Bird, reflective mirror, solid colored balls, etc.
  • Materials to sort (including items to sort by color, shape, and/or specific salient features, e.g. variety small plastic animals)
  • Black gloves with light up tips (from http://www.flashingblinkylights.com)
  • Red and yellow balloons or Light-Up Balloons (or balloons with finger lights inside)
  • Several multicolored/patterned toys/objects that emit sound
  • Slap bracelets (in reflective red and/or yellow) or sequined headbands to use as Koozie
  • Red light up “sunglasses” (from http://www.flashingblinkylights.com)
  • Black apron or black smock
  • 4 separate pieces of fabric ranging in pattern complexity including; plain black, slight pattern, moderate pattern, complex pattern
  • iPad with apps (applications commonly used with students with CVI)
  • Container of Lysol wipes
  • Extension cord(s)
  • Measuring tape
  • Extra Batteries (AA and AAA)
